Johnathan Swofford NP
Johnathan Swofford NP is located in Decatur Texas and has graciously partnered with Axtell Pharmacy to provide services to our patients. He graduated with a 4.0 from Walden University’s Nurse Practitioner Program and holds a Bachelors of Science in Nursing from The University of Texas at Arlington. His commitment to continuous learning is evident through certifications such as FNP-C, CPR, ACLS, and PALS. Beyond the medical field, Johnathan finds joy in travel, quality time with family and the calming pursuit of fishing. His multifaceted career and dedication to both clinical excellence and community service exemplify a holistic approach to healthcare.
Johnathan is currently serving as the Nurse Practitioner at Array Primary Care and brings expertise in primary care. His journey includes roles as a 360 Department Nurse Practitioner and in the Transitional Care Clinic. Johnathan’s early career involved pivotal roles as a Flight Nurse and Paramedic at CareFlite and as a Firefighter/paramedic with the Flower Mound Fire Department. He has also contributed significantly as the EMS Chief at Parker County ESD #1.

Dusty Rhoades NP
I was raised here in Sulphur Springs, and recently moved back about a year ago from Sherman, Texas. I have two girls, both are grown and still reside in Sherman. My oldest daughter is married with one daughter of her own and is currently in nurse practitioner school. My youngest is a cosmetologist at a very successful salon in Allen, Texas, living her best life. I am very proud of both of my girls and would not be where I am today without their love and support. I am married to Jason Strange, and we live on a little slice of heaven just outside of town on 154 with all our fur and feather children. I am a God-fearing Christian, and I give him all the glory for my success and education. He has brought me through more than I care to remember and blessed me far more than I will ever deserve!
As a provider, you must encompass a level of professionalism. This means possessing certain qualities and traits; honesty, integrity, having morals, and certain ethical standards in your everyday practice. Patients must be able to trust you with their health care, and integrity breeds trust. Patients are unable to receive adequate healthcare without a certain level of trust. They must know their provider is always going to do what is in their best interest, even when it is not easy to do so. It may not always be the answer a patient wants to hear, or the easiest path, but it will adhere to an above standard level of care, one that is safe. A standard of care that I would in turn provide to my own family. That is the kind of care you will always receive from me and my staff here at Rhoades Family Practice.

Cameron Boyers is a skilled and compassionate Family Nurse Practitioner dedicated to delivering high-quality, patient centered care to individuals and families. As an MSN-FNP graduate from Angelo State University and an AANPCB certified provider, Cameron brings more than 10 years of experience as a Registered Nurse Practitioner, specializing in family practice, adult and geriatric care and palliative support. His approach combines advanced clinical expertise with a genuine commitment to building trusting relationships with patients, promoting wellness, and managing chronic and acute conditions. Cameron's extensive background in family medicine and in a rehabilitation setting includes proficiency in a wide range of clinical skills. A vital member of Array Health in Decatur Texas, Cameron is dedicated to fostering a warm, welcoming environment where patients feel supported in their health journey.
